NEW CLIENTS Please arrive 10 minutes before your first appointment so that we can ensure your forms are complete and your direct billing is set up (if applicable).

LOCATION Inside Revamp Clinic at 4984 Dundas St W, Toronto, ON M9A 1B7; North side;

ARRIVAL The clinic is located inside Revamp Clinic; Please use the 3rd door from the left, located under Revamp; Ring bell, and someone will open the door

INTAKE FORMS When you book an appointment, you will receive a welcome letter in your email with a prompt to complete the intake forms. If you have not received an intake form, please contact us to send them to you. Existing clients: we require an update on your health history once a year.

CANCELLATION POLICY We ask that you respect our therapists’ time, so please give us at least 12 hours notice in advance if you wish to cancel or reschedule your appointment.

PAYMENT We accept cash and e-transfers

RMT RECEIPT An RMT receipt will be issued once payment is received.
